Job Summary
Provide direct patient care under RN supervision, implementing the plan of care and performing treatments while collecting assessment data for the care team. Deliver accurate documentation, coordinate home health services with OT, PT, and dieticians, and report identified needs to the registered nurse and physician. Utilize equipment and supplies effectively, participate in professional growth, and perform other duties as assigned by the registered nurse. Maintain a licensed driver's status with an insured vehicle and current CPR certification. Monday–Friday schedules with on-call rotation are available for full-time or part-time roles.
Required Qualifications
- Completion of an accredited practical (vocational) nursing program
- Licensed in the state where the agency operates
- Currently licensed as a Licensed Practical Nurse or Licensed Vocational Nurse in the state
- Must be a licensed driver with an insured and reliable automobile
- Maintains current CPR certification
Desired Qualifications
- Three years nursing experience
- Community/home health or medical-surgical experience
