Job Summary
Assess residents on admission and ongoing basis, communicating significant condition changes to the interdisciplinary team. Accurately document assessments and participate in care planning, updating plans when interventions indicate revision. Implement nursing and physician orders, including IV fluids if certified, ensuring timely medication administration and treatments. Monitor residents closely to identify behavioral changes or untoward symptoms, answering call lights and assisting with ADLs. Use SBARR for physician notifications and apply critical thinking during crises or staff disputes. Maintain composure in challenging situations and address problem areas using the QAPI process. Complete at least twelve hours of nursing-related continuing education annually.
Required Qualifications
- Education from an accredited school of nursing that results in becoming a Licensed Practical Nurse
- Current license in the state you will be working as a Licensed Practical Nurse
- IV Certification required or willing to obtain within six months of employment
- Current CPR/AED Certification required or willing to obtain within six months of employment
- Must be able to assess the basic nursing care needs of the geriatric resident
- Must possess excellent communication skills to interact with residents, visitors and staff
- Excellent time management and organizational skills are required
- Must possess critical thinking skills
Desired Qualifications
- Long-term care/Rehab/Assisted Living experience preferred
