LPN Rehab Nurse Full-Time 12-Hour Night Shift

Provides nursing care for geriatric residents in a long-term care/rehab setting. Responsibilities include assessing residents on admission and ongoing basis, updating and communicating care plans to the IDT and families, administering medications and treatments safely and timely, performing nursing interventions per care plan, documenting assessments and interventions, utilizing SBARR for physician communication, maintaining composure during crises, monitoring care plan delivery, identifying and addressing problem areas via QAPI, responding to call lights and assisting with residents' ADLs, and pursuing ongoing nursing education. Requires LPN license, IV and CPR/AED certifications (or willingness to obtain within specified timelines), and ability to assess basic nursing needs and communicate effectively. 12-hour night shift; pay range $26.96-$43.95 per hour.
