LPN Rehab Nurse Full-Time 12-Hour Night Shift

Provide basic nursing care by assisting in resident assessments on admission and ongoing, accurately documenting all findings and communicating significant changes to the interdisciplinary team. Implement nursing interventions relevant to the care plan, including medication administration and treatments, while using critical thinking to identify behavioral changes or untoward symptoms. Monitor resident care to ensure effective delivery of the plan, answer call lights, and assist with activities of daily living as needed. Notify physicians timely using SBARR communication and utilize the QAPI process to address problem areas. Maintain composure during crises and complete twelve hours of nursing-related continuing education annually.
