LPN Rehab Nurse

Provides basic nursing care for geriatric residents in a long-term care/rehab setting, including assessing residents on admission and ongoing, communicating changes to the interdisciplinary team, documenting assessments, and updating the care plan. Delivers nursing interventions per the plan of care, administers medications and treatments safely, and maintains compliance with IV certification, SBARR communication, and QAPI processes. Participates in care planning, educates residents and families, assists with ADLs, responds to call lights, and supports ongoing nursing education. Maintains composure during crises and ensures timely documentation and safety in delivery of care. May be assigned other duties as appropriate.
