Job Summary
LPN RAI Coordinator - PT - Day at Heritage Inn (Statesboro, GA). Facilitates communication for assessment/care plan processes, supports Director with patient interviews and evaluation of conditions, assists in developing comprehensive care plans, coordinates schedules with nursing center staff, and ensures care plans meet regulatory timelines. Requires an active Georgia LPN license, completion of an LPN program or nursing degree, proficiency with Microsoft Office, and strong communication and clerical/organizational skills to collaborate with interdisciplinary teams and family members. Experience with MDS processes and care-plan coordination is beneficial.
Required Qualifications
- Nursing degree or graduate of an approved LPN program
- Active LPN license to practice in the State of Georgia
