LPN - Queens NY

Conduct advanced clinical home care services for patients recovering from illness, injury, or surgery, including patient assessment, medication management, and patient/family education. Perform skilled revisits to patients based on clinical need, implementing the plan of care and evaluating treatment progress. Accurately observe, measure, record, and report clinical data related to patient health status, while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications, and perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning. Collect and communicate signs of health decline to the RN supervisor. Requires NY State LPN license, one year of med/surg experience, and strong computer skills.
