Job Summary
Provide direct patient care as an LPN in a healthcare, long-term care, or assisted living setting, including administering medications, wound care, and assisting with feeding, dressing, and mobility. Monitor residents’ vital signs, document treatments and responses, and report concerns to supervising RNs/physicians. Maintain up-to-date care documentation, follow individualized care plans, and collaborate with RNs, physicians, and other healthcare professionals to ensure cohesive patient care. Ensure infection control and safety protocols, advocate for residents’ needs, and support emotional well-being. Proficient in EMR/nursing documentation, with strong communication and teamwork skills. Qualifications include completion of an accredited LPN/LVN program and an active LPN license; experience in healthcare or long-term care preferred but not required; new graduates welcome.
