LPN - PRN - Home Health

Assess and assist with managing patients' total needs following physician orders with RN oversight. Perform routine nursing procedures, coordinate care with the RN and other team members, and collaborate to develop and revise individual plans of care. Serve as a patient educator to foster self-care and report safety concerns to leadership. Complete required documentation within established timeframes, participate in care team conferences, and check messaging daily. Rotate through call shifts and weekend visits as needed. Act as a preceptor for new LPN employees and nursing students. Attend continuing education, in-services, and staff meetings to maintain current clinical knowledge. Comply with all Wellstar Health System policies and standards of work.
