LPN - PRN - Home Health

Assess and assist with managing patients' total needs following physician orders with RN oversight. Perform routine nursing procedures, coordinate care with the RN and other team members, and foster self-care for home patients. Document accurately, participate in care team conferences, and serve as a preceptor for new LPN employees. Attend staff meetings, in-services, and special projects while completing call rotation and weekend visits as needed. Maintain current clinical knowledge through continuing education and comply with all Wellstar Health System policies.
