LPN - PRN - Home Health

Assess and manage total patient needs following physician orders with RN oversight, performing routine nursing procedures to meet physical, emotional, and developmental requirements. Coordinate care and services with the RN and other team members, developing and revising individual plans of care based on patient needs and physician orders. Serve as a patient educator to foster self-care for home patients and participate in care team conferences, voice mail checks, and secure messaging daily. Assist in the orientation and education of new employees and nursing students while attending continuing education and in-services. Participate in call rotation and weekend visits as needed, maintaining accurate documentation within established timeframes. Minimum 1 year medical/surgical nursing experience in an acute care environment preferred.
