Job Summary
Provide professional nursing care for residents of all ages in long-term care under the supervision of a registered nurse, advanced practice provider, or physician. Ensure the health, comfort, and safety of residents by administering medications or treatments as ordered, performing testing to evaluate resident status, and responding effectively to emergent situations. Contribute to resident assessments, document interactions and care outcomes, and counsel residents, family members, and advocates on prevention and treatment plans. Work effectively as a member of an interdisciplinary care team while maintaining confidentiality and completing required annual continuing education and competency training.
Required Qualifications
- Graduate from an accredited practical nursing program as a Licensed Practical Nurse
- Associate degree in practical nursing
- Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum
- Minimum of 400 hours of experience as LPN at another facility
- Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board
- Possess multi-state licensure privileges
- Obtains and subsequently maintains required department specific competencies and certifications
- Completes required annual continuing education units (CEU) and competency training requirements of the department and facility
