Job Summary
Provide hands-on patient care and education, administer medications and treatments per orders, and document all visits and MARs accurately within 24 hours. Report any changes in patient condition to the supervising RN and adhere to infection control and safety protocols while using the EVV system for visit verification. Work under RN supervision in accordance with the Connecticut Nurse Practice Act and agency policies. This private duty per diem assignment is Monday–Friday, 8:30 AM–4:30 PM.
Required Qualifications
- Active, unrestricted Connecticut LPN license
- Graduate of an accredited practical nursing program
- Reliable vehicle
- Valid driver's license
- Auto insurance

- 1+ year of home health or related experience
