Job Summary
Provide skilled nursing care to assigned patients in private homes, utilizing assessment, planning, implementation, and evaluation per physician orders and nursing care plans. Perform specific treatments and medication administration while observing aseptic technique, and identify new problems or status changes for Director/Supervisor and physician notification. Document all interventions, orders, and clinical observations accurately in medical records and adhere to agency policies and MGA standards. Communicate significant patient changes to the Director of Clinical Services and collaborate with the registered nurse team under supervision.
Required Qualifications
- Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education
- Valid/unencumbered verification/license to practice the job as described in the state where services are provided
- Professional experience as required per state regulations
- Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater
- Reliable transportation
- Current TB results or Chest x-ray
- Current CPR certification
- Ability to exercise independent and sound judgment
- Ability to regularly, sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull and reach with hands and arms
- Vision and Hearing capabilities within normal limits, with or without aid
- Ability to frequently lift and or move up to 15 pounds
- Ability to occasionally lift or move 100 pounds or more if applicable
- Ability to perform the anticipated level of physical assist required to provide safe client care
- Ability to travel frequently to complete home health visits
