Job Summary
LPN in Saint Luke’s Population Health Division with a Monday–Friday day-shift schedule. The role centers on coordinating care for low-to-moderate risk patients post-discharge as an Outreach Specialist. Responsibilities include calling patients, planning and executing transitions of care follow-up, applying LPN clinical knowledge to optimize health management, and working independently to schedule and influence patient care with high visibility within a faith-based, nonprofit health system. The position supports a hybrid work arrangement (3 days in-office per week after a 90-day period) at Saint Luke’s Primary Care Clinic locations in Independence, MO or Platte City, MO, with no night or weekend requirements. Key attributes include ability to operate with autonomy, strong patient communication, and a commitment to high-quality patient care within a large acute-care network.
Required Qualifications
- Licensed Practical Nurse (MO) license
- NURSYS (National Council of State Boards of Nursing)
- 3-5 years of LPN experience
