LPN - Population Health

LPN in Saint Luke’s Population Health Division with a Monday–Friday day-shift schedule. The role centers on coordinating care for low-to-moderate risk patients post-discharge as an Outreach Specialist. Responsibilities include calling patients, planning and executing transitions of care follow-up, applying LPN clinical knowledge to optimize health management, and working independently to schedule and influence patient care with high visibility within a faith-based, nonprofit health system. The position supports a hybrid work arrangement (3 days in-office per week after a 90-day period) at Saint Luke’s Primary Care Clinic locations in Independence, MO or Platte City, MO, with no night or weekend requirements.
