Job Summary
Administer medications and treatments as prescribed, perform triage assessments, and monitor patient vital signs. Collaborate with healthcare team members to develop and implement patient care plans while educating residents and families on disease processes. Maintain accurate patient records and conduct supervisory visits in clients' homes regarding Certified Nursing Assistants/Personal Care Technicians. This role includes one weekend per month and requires a current LPN license in the state of practice.
Required Qualifications
- Current LPN license in the state of practice
- Strong communication skills for effective interaction with patients and families
- Ability to perform clinical procedures competently
- Compassionate demeanor with a commitment to patient-centered care
- One weekend per month (Saturday and Sunday) 8:00 AM - 8:00 PM
Desired Qualifications
- Proven experience in a skilled nursing setting preferred
