Job Summary

Administer treatments and medications per MAR, TAR, and PRN sheets; assemble equipment including catheters, tracheotomy tubes, and oxygen suppliers. Assist residents with activities of daily living, review 24-hour reports, and complete unfinished orders from the previous shift. Document resident care changes, maintain appointment calendars, and ensure annual physician evaluations. Assess resident conditions, notify physicians or families of changes, and receive/receive physician orders while complying with Department of Human Services regulations. Complete lab requests and ensure all necessary paperwork and RASP forms are finalized. Full-time position with 2:30 PM to 11:00 PM shifts, every other weekend and holidays, at The Glen PC and Meadow Ridge in Lancaster and Willow Street, PA.

Required Qualifications

  • Active Pennsylvania Licensed Practical Nurse (LPN) license
  • Administers treatments and medication as specified in MAR, TAR, PRN sheets
  • Assembles and uses equipment including catheters, tracheotomy tubes and oxygen suppliers
  • Assists with providing guidance and assistance to Residents as needed with ADLs
  • Reviews 24 hour report and general notes in PCC from the preceding shift
  • Follows through on unfinished orders/tasks from shift prior
  • Assures Resident care is completed by assignments
  • Documents changes in Resident care and condition
  • Maintains calendar of Resident appointments and assures that transportation of Residents to appointment
  • Assesses Residents' condition and informs physician, family/responsible party of any changes and plan of action/treatment
  • Receives orders from physicians and brackets order with time, date and initials
  • Transfers order to PCC
  • Notifies pharmacy of changes in medication orders or need for refills
  • Complies with Department of Human Services regulations and follows the WVC policies and procedures
  • Completes lab requests per physician orders with all pertinent information
  • Assures that Residents are seen by a physician annually for medical evaluation or when change in Resident's condition
  • Assures a RASP is completed with each DME
  • Assures that physicians complete necessary paperwork
  • Available for every other weekend and holiday shifts
