Job Summary
Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled needs, implement care plans, and document visits electronically within 24-48 hours. Accurately observe, measure, record, and report clinical data while reconciling and administering prescribed medications. Execute nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning under RN direction. Collect and communicate signs of health decline immediately. Requires NY State LPN license, one year med/surg experience, driver's license, and car.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
