Job Summary
Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform revisits to patients based on skilled need, implement care plans, and evaluate treatment outcomes. Accurately observe, measure, record, and report clinical data while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning as indicated in physician plans. Maintain NY State LPN licensure with at least one year of med/surg experience, a driver's license, and vehicle.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Must have a driver license and car
- Good computer skills
