Job Summary
Licensed Practical Nurse (LPN) for pediatric home health in Reading, PA. Provide direct, one-on-one medical care to infants, children, and/or adolescents in patients' homes according to the plan of care. Responsibilities include ongoing patient care and assessments, administration of prescribed medications and therapies, coordination of care, educating families on patient clinical care to improve outcomes, and implementing preventative measures to maintain high-quality care. Required Qualifications
- Active LPN/LVN license
- Licensed Practical Nurse (LPN)
- Pediatric home health experience preferred
- Ability to provide care in a client home setting
