LPN Patient Services Coordinator

Manage schedules for all patients, including supervisory visits, discharge visits, OASIS collections, and transfers to inpatient facilities. Reschedule declined or missed visits, prepare weekly agent schedules, and perform initial productivity reviews. Assist with weekly case conferences, receive clinical questions from field staff, and refer issues to the Branch Director. Maintain the client hospitalization log, verify visit paper notes, and run scheduling reports. May perform patient visits and participate in on-call rotation.
