LPN Patient Care Specialist

Provide direct patient care aligned with personalized care plans and physician orders. Monitor vital signs, symptoms, and responses to treatments while collaborating with Registered Nurses to report findings and execute care plans. Cultivate meaningful relationships with patients by understanding their physical, mental, and emotional needs to support successful recovery and celebrate their victories. Requires current LPN/LVN licensure, CPR certification, and one year of experience in an inpatient medical-surgical or general hospital setting, with rehabilitation experience preferred.
