LPN Patient Care Manager - Home Health

Respond to phone inquiries from field staff, physician offices, and patients; monitor incoming faxes and track pending lab results. Coordinate communication with appropriate personnel and transmit relevant documents to physician offices. Conduct follow-up actions based on health check calls or tele-monitoring alerts and compile patient lists for weekly case conferences. Support Clinical Manager in managing software system reports and assist with payer source authorization processes. Deliver direct patient care within scope of practice and per physician orders, ensuring compliance with regulatory guidelines and episode management protocols. Complete required learning modules and state-specific training as mandated.
