LPN - Paterson NJ

Conduct patient assessment, medication management, and patient/family education for home care recovery. Perform revisits to patients based on skilled need, implementing the plan of care and evaluating treatment outcomes. Document visits electronically within 24-48 hours and record clinical data related to health status. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Collect and report signs of health decline accurately. Requires NY State LPN license, one year Med/surg experience, and good computer skills.
