LPN - Paterson NJ

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education. Perform skilled patient revisits based on clinical need, implement and evaluate care plans, and document visits electronically within 24-48 hours. Accurately observe, measure, record, and report clinical data while administering prescribed medications and performing nursing treatments such as urinary catheterization and sterile dressing changes under RN direction. Collect and communicate signs of health decline immediately. Requires NY State LPN license, one year med/surg experience, and good computer skills.
