Job Summary

Provide direct patient care, including administering medications, wound care, and monitoring vital signs. Assist in developing individualized care plans with RNs and physicians while supporting patients with daily living activities. Document patient information accurately in Hospice Tools systems and educate families on treatment plans. Respond promptly to patient needs, escalating concerns when necessary, and maintain a clean, safe environment. Requires current Minnesota LPN license, clinical experience, and proficiency in Hospice Tools. Required Qualifications

  • Current LPN license in Minnesota and in good standing
  • Proven experience in a clinical, hospital, or long-term care setting
  • Proficiency in Hospice Tools systems and basic medical technology
