Job Summary
Implement care continuously for assigned patients, administering medication therapy and vaccines as ordered by the provider. Perform and assist patients with treatments and procedures, while documenting patient responses to teaching in the medical record. Observe, report, and promptly document significant physical, psychological, or safety status changes to the RN, PSC, or provider. Communicate patient care needs to ancillary staff and provide health education based on individual requirements. Demonstrate awareness of cultural backgrounds and offer translation services as needed. Maintain BLS certification and hold a valid NYS LPN license with 1-2 years of experience in hospital, med/surg, pediatrics, OB/Gyn, or ambulatory care.
Required Qualifications
- Graduate of an accredited school of nursing
- 1-2 years of experience in hospital based nursing, Med/Surg, Pediatrics, OB/Gyn
- Ambulatory care experience
- Willingness to devote the time required completing assigned tasks on schedule
- NYS LPN License
- Bilingual in languages based on site's needs
- Basic Life Support (BLS) Certification through the American Heart Association
- Effectively communicate with all levels of the organization
