LPN PACE

Provides nursing care to participants under the direct and indirect supervision of the registered nurse(s) and providers of primary care. Responsibilities include scheduling medical appointments, performing nursing assessments, wound care, medication administration, obtaining lab specimens, administering EKGs, completing documentation in medical records, conducting home visits, patient and caregiver education on conditions and treatments, participating in QA/IP programs, and coordinating 24-hour care delivery. The role involves triaging after-hours care, communicating changes to the interdisciplinary team, and ensuring adherence to approved plans of care.
