Job Summary
Licensed Practical Nurse (LPN) in Home Health providing clinically skilled care in patients' homes within the agency's care plan. Responsible for high-quality care within state practice scope, thorough documentation in electronic medical records, active participation in care coordination and timely communication with patients, families, physicians, and case managers. Must adapt to diverse home environments and demonstrate independent work capability, effective communication, and organizational skills. Offers a comprehensive benefits package and a compensation structure including hourly pay and per-visit compensation for visits, with full-time productivity targets. Includes emphasis on infection control, adherence to plan of care, and compliance with regulatory requirements.
Required Qualifications
- Current and unrestricted LPN licensure in state of practice
- Current driver''s license and vehicle insurance, access to a dependable vehicle, or public transportation
- Ability to work flexible hours as required to meet identified client needs
- Good communication, writing, and organizational skills
