Job Summary
Manage and facilitate timely patient transfers to home health, DME, and infusion by receiving referrals and physician orders from social services, case managers, nurses, and physicians via hospital EMR. Document this information in the home health electronic medical record and communicate it to appropriate personnel. Perform ICD coding for all referrals and obtain necessary face-to-face documentation through internal EPIC systems or outside referrals. Requires a Virginia or West Virginia LPN license or eligibility, with experience in home health or medical/surgical nursing preferred.
