Job Summary
LPN on the Clinical Care Team providing direct patient care and supporting providers. Responsibilities include preparing patients for visits (vitals, height/weight, medication reconciliation, allergy and history checks), lab work and in-house procedures (EKG, strep, urine, pregnancy tests, A1C), medication management, vaccine administration, charting in the EHR, ensuring proper documentation and compliance with HIPAA, conducting recalls and health maintenance activities, preparing rooms, restocking supplies, and participating in QAPI and staff meetings. Requires NY LPN licensure, EMR experience, attention to detail, and effective written and verbal communication.
Required Qualifications
- Graduate of an accredited program for Licensed Practical Nurse with a current valid license to practice nursing in New York State
- One year of experience in a nursing role preferred
- Experience with EMR systems
- Ability to work with a diverse patient population
- Excellent attention to detail
- Ability to communicate both written and verbally
