LPN

LPN will be part of the Clinical Care Team providing direct patient care and supporting providers. Responsibilities include preparing patients for visits (vital signs, medications reconciliation, allergies, social history), coordinating pre-visit planning with software tools and chart review, rooming and assisting with procedures and screenings, performing in-house labs and basic procedures (EKG, peak flow, etc.), preparing lab specimens, processing medication refills, administering vaccines and injections, handling patient communications, maintaining exam rooms, stocking supplies, updating patient charts and documentation in the EHR, and participating in infection control and QAPI activities. Qualifications emphasize graduation from an accredited LPN program, current NY LPN licensure, EMR experience, attention to detail, and effective communication; one year of nursing experience is preferred.
