Job Summary

Provide direct patient care to residents by administering medications, performing wound care, and assisting with personal hygiene, feeding, and mobility. Monitor vital signs such as temperature, blood pressure, and pulse, documenting all findings and reporting abnormalities to supervising staff. Collaborate with RNs, physicians, and interdisciplinary teams to execute individualized care plans and participate in care conferences. Maintain accurate documentation of treatments, medication administration, and resident progress while adhering to infection control protocols and safety guidelines. Act as an advocate for residents, offering emotional support and ensuring their dignity and comfort are prioritized throughout their stay.

Required Qualifications

  • Completion of an accredited LPN or LVN program
  • Current and active Licensed Practical Nurse (LPN) license
  • Strong clinical and patient care skills
  • Excellent communication and interpersonal abilities
  • Ability to work well as part of a team and collaborate with other healthcare professionals
  • Compassionate and patient-focused approach to care
  • Proficient in electronic medical records (EMR) systems and nursing documentation

Desired Qualifications

  • Previous experience in a healthcare or long-term care setting
