Job Summary
As a Nurse Care Manager, you will provide health education to clients in individual and group settings, monitor prescriber scheduling, review lab work, document nursing activities, administer medications including injectables, and provide emergency medical care when necessary. You will also coordinate client care with internal and external providers, support prescribers with clerical tasks, and perform other duties as assigned. Candidates must be a licensed practical nurse (LPN) with a high school diploma and a valid driver's license.

- Valid Driver's License
