Job Summary
Provide in-home direct patient care according to the prescribed plan of care, monitor progress, administer medications and IV fluids, inspect and care for wounds, document changes, and collaborate with providers to ensure optimal patient outcomes. Requires an LPN license, CPR/AED/BLS/First Aid certifications, and reliable transportation; practical trach/ventilator experience is preferred but not required. The role supports flexible scheduling (FT/PT/PRN) and focuses on delivering compassionate, dignified care in patients’ homes.
Required Qualifications
- Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
