LPN - Newark NJ

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under the direction of a Coordinator of Care RN or RN. Perform revisits to patients based on skilled need, implement plans of care, and evaluate treatment progress. Accurately observe, measure, record, and report clinical data related to health status, while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning as indicated in the physician's plan. Maintain accurate documentation of any signs or symptoms indicating a decline in patient health status. Requires NY State LPN license, minimum one year Med/surg experience, and good computer skills.
