LPN - Newark NJ

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under the direction of an RN. Perform revisits to patients based on skilled needs, implementing the plan of care and evaluating treatment progress. Document visits electronically within 24-48 hours and accurately reconcile and administer prescribed medications. Observe, measure, record, and report clinical data related to patient health status, including communicating signs or symptoms of decline. Perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning as indicated in the physician's plan. Requires NY State LPN license, one year med/surg experience, and good computer skills.
