Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform revisits to patients based on skilled needs, implement care plans, and evaluate treatment progress. Document visits electronically within 24-48 hours and accurately record clinical data, symptoms indicating health decline, and administered medications. Execute nursing treatments such as urinary catheterization and sterile dressing changes per physician plans. Requires NY State LPN license, one year med/surg experience, valid driver's license, and car.
