LPN - New York NY

Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled need, implementing care plans and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning under RN direction. Collect and communicate signs of health decline immediately. Requires NY State LPN license, one year med/surg experience, driver's license, and good computer skills.
