Job Summary
Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform revisits to patients based on skilled need, implementing the plan of care and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data related to patient health status, documenting visits electronically within 24-48 hours. Administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes. Requires NY State LPN license and minimum one year Med/surg experience.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Good computer skills
