Job Summary
Conduct advanced clinical home care services for patients recovering from illness, injury, or surgery, including patient assessment, medication management, and patient/family education. Perform revisits to patients based on skilled needs under the direction of a Coordinator of Care RN, implementing the plan of care and evaluating treatment outcomes. Accurately observe, measure, record, and report clinical data related to patient health status, including documenting signs of decline outside established parameters. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning as indicated. Document visits electronically within 24-48 hours. Requires NY State LPN license, minimum one year med/surg experience, and good computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Good computer skills
