Licensed Practical Nurse (LPN) at MJHS Home Care in Brooklyn, NY with a $10,000 sign-on bonus or student loan assistance. Provide advanced clinical home care services, including patient assessment, medication management, and patient/family education, to patients recovering from illness, injury, or surgery. Under the direction of a Coordinator of Care RN, perform revisits to patients at home, implement and evaluate the plan of care, and document visits electronically within 24-48 hours. Observe, measure, and report health status, reconcile and administer prescribed medications, and perform nursing treatments (e.g., urinary catheterization, sterile dressing changes, tracheal suctioning) as outlined in the physician’s plan of care. Requires NY State LPN license, minimum 1 year Med/surg experience, valid driver’s license and car, and good computer skills.
