LPN

Licensed Practical Nurse (LPN) for MJHS Home Care provides advanced clinical home care services, patient assessment, medication management, and patient/family education under the direction of a Coordinator of Care RN. You will perform home revisits to patients, implement and evaluate the plan of care, and document visits within 24-48 hours. Responsibilities include observing and recording clinical data, reconciling and administering prescribed medications, and performing nursing treatments such as urinary catheterization, sterile dressing changes, and tracheal suctioning according to the physician’s plan of care.
