LPN

Provide advanced clinical home care services including patient assessment, medication management, and patient/family education under the direction of a Coordinator of Care RN. Perform revisits to patients based on skilled need, implement plans of care, and evaluate treatment outcomes. Document visits electronically within 24-48 hours and accurately record clinical data, reporting any signs of health decline. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes. Requires NY State LPN license, one year med/surg experience, driver's license, and car.
