Conduct advanced clinical home care services for patients recovering from illness, injury, or surgery, including patient assessment, medication management, and education. Perform revisits to patients based on skilled need under the direction of an RN, implementing and evaluating the plan of care. Accurately observe, measure, record, and report clinical data, documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes. Maintain a valid NY State LPN license, one year of med/surg experience, a driver's license, and good computer skills. Earn up to $74,000 annually with a sign-on bonus or student loan assistance.
