Job Summary
Conduct skilled home care visits for patients recovering from illness, injury, or surgery under RN direction. Perform patient assessments, administer prescribed medications, and execute nursing treatments such as urinary catheterization and sterile dressing changes. Reconcile medication plans, observe and report clinical data regarding health status, and document visits electronically within 24-48 hours. Evaluate treatment plans and provide patient and family education. Requires NY State LPN license, minimum one year med/surg experience, and good computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Good computer skills
