Job Summary
Deliver individualized clinical care to residents with cognitive impairment, maintaining a calm, structured, and compassionate routine. Safely administer medications, treatments, and perform skin care/wound documentation per physician orders and Indiana state regulations. Observe condition changes, conduct routine health assessments, and promptly report updates to the Charge Nurse, Director of Nursing, and families. Provide clinical direction and mentorship to Resident Care Assistants on the Memory Care floor while collaborating with interdisciplinary teams to communicate care plans. Required Qualifications
- Active, unencumbered license as a Licensed Practical Nurse (LPN) in the State of Indiana (or compact state equivalent)
- Current CPR / BLS certification
- Exceptional patience
- Strong clinical assessment abilities
- Crisis de-escalation techniques
- Warm, empathetic communication style
Desired Qualifications
- Prior LPN experience in Assisted Living, Memory Care, or Long-Term Care
- Experience working with Alzheimer's or dementia care
