Job Summary
Administer medications to clients, including injections, while providing care management and coordination services for adult clients in the medication clinic. Engage individuals and families as partners to avoid and resolve crisis situations, offering education and support to clients and their support persons. Coordinate care with prescribing physicians, nurse practitioners, and external treatment providers, and assist with scheduling and inventory management for medical supplies. Prioritize urgent needs by returning phone calls to determine care levels and develop access plans, while working with the pharmacy to coordinate prescription assistance. Complete all documentation and reporting requirements thoroughly and timely.
Required Qualifications
- Must be licensed in Kansas by the Kansas Board of Nursing as a Licensed Practical Nurse.
- Familiarity with mental health theory, emerging promising practices and Evidence-Based practices.
- Knowledge of medical and non-medical services delivery system.
- Effective communication with clients, family and supports, guardians and other services providers.
- Ability to establish relationships with individuals and organizations of influence within the social services community.
- Demonstrated strong interpersonal and communication skills.
- This position will be based at the 47th Street location.
- Transportation from other Wyandot BHN locations or at various community sites required to meet the business needs of the clients.
Desired Qualifications
- Prior experience with mental health is preferred.
