LPN Med Care Manager PT

Administer medications and treatments per physician orders while following the Six Rights of Medication Administration. Document in MAR/TAR accurately and in real time, monitor symptoms for relief, and report refusals or adverse reactions promptly. Conduct Service & Health Updates, transcribe physician orders, and observe physical, emotional, or cognitive changes. Maintain medication carts, complete narcotic counts, and uphold compliance standards to prevent medication errors and protect resident safety. This role requires an active LPN license, CPR and First Aid certification, and strong assessment skills within a senior living environment focused on quality of life.
