LPN Med Care Manager PT

Administer medications and treatments per physician orders while following the Six Rights of Medication Administration. Document in MAR/TAR accurately and in real time, monitor resident symptoms, and report refusals or adverse reactions promptly. Conduct Service & Health Updates, transcribe physician orders, and maintain medication carts with strict compliance standards. Identify physical, emotional, or cognitive changes early to protect resident safety and prevent medication errors. Requires an active LPN license, CPR and First Aid certification, and strong assessment skills.
